Refer a Participant to Comfort Pathways
We accept referrals from participants, families, GPs, LACs, allied health professionals, hospital teams, justice and corrective services, and Public Guardians. Our team is dedicated to responding to every new enquiry within one business day to ensure timely support.
WHO CAN REFER
We Welcome Referrals From Everyone
You do not need to be a professional to refer someone to Comfort Pathways. If you know a participant who needs support, whether you are a family member, a GP, or a hospital discharge planner, we want to hear from you.
Participants
You are welcome to refer yourself directly. No professional involvement or GP referral is needed. Simply get in touch and we will take it from there.
Families & Carers
If you are a carer or family member who can see a participant needs more support, you can refer on their behalf. The participant remains at the heart of our practice.
GPs & Medical Professionals
We work alongside nurses, GPs and medical specialists who identify patients needing NDIS coordination or support following hospital stays.
Hospital Social Workers & Discharge Planners
We specialise in complex hospital to community transitions and accept referrals from hospital teams. We provide a quick response for urgent discharge needs.
Allied Health Professionals
We welcome referrals from OTs, psychologists, physios and speech pathologists who are working with participants needing stronger coordination or stable housing support.
LACs & Support Coordinators
If you are an LAC or a coordinator needing a specialist partner for a complex case, or if you are transitioning a participant, we would value your referral.
Justice & Custodial Transition Services
We support NDIS participants transitioning from custodial settings into the community. We work closely with corrective services, justice health, and community corrections teams to ensure continuity of supports, appropriate risk management, and coordinated community integration from day one.
Guardianship & NCAT
Our coordinators have experience supporting participants through guardianship arrangements and NCAT processes. We liaise with Public Guardians, the NSW Trustee, legal representatives, and relevant stakeholders to ensure supports are coordinated, appropriate, and compliant with any guardianship orders in place.

We Review Your Referral

We review every referral on the day it is received and confirm receipt within 1 business day.
We reach out to the referrer and participant to understand the situation and the urgency.
We are honest about whether we are the right fit. If we cannot help, we will point you to a peer.
The service agreement is started and an intake meeting is arranged to begin support quickly.
For crisis referrals, please call us directly rather than waiting for an online form response.
